Via Christi HOPE: Help for families of seniors

Senior careWhen Rhonda Hicks first realized her mother, Hazel Thomas, was suffering from dementia, she took over her care, but had little idea what she was getting into.

Hazel had several different doctors due to her medical condition and her daughter quickly realized she was unprepared for all the coordination her mother’s care would entail.

Luckily for Rhonda and other families in similar situations in the Wichita area, Via Christi HOPE is there to help.

Kansas’ first Program of All-Inclusive Care for the Elderly or PACE®, Via Christi HOPE provides the following services:

  • Day center
  • On-site physician visits
  • Physical therapy
  • Medications
  • Social services
  • Transportation to medical appointments
  • Inspections and equipment to make participants’ homes safer
  • A home health agency to serve clients’ needs

You can read more about Via Christi HOPE in the latest issue of Via Christi Life, a new magazine focusing on real patient stories and providing examples of how Via Christi Health is leading the way in the transformation of health care.
